Community Liaison 
Abbey Hospice has a job opening for the position of Community Liaison. The applicant must be a licensed Registered Nurse (RN) familiar with the hospice care industry. This position requires the ability to adapt to changing daily activities and schedule. Excellent people skills are required for developing and maintaining relationships with referral sources. Customer service qualities are necessary for problem prevention and resolution.

Chaplain--Full Time
Abbey Hospice is seeking a full-time, Staff Chaplain that is responsible for the provision of spiritual care services to patients and families/caregivers throughout the Abbey service area centered in Social Circle.  
 
Position-Specific Skills, Competencies and Responsibilities:
Assures spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
Facilitates the development in the interdisciplinary team/group plan of care to meet identified spiritual needs.
Provides direct spiritual care to patients and families/caregivers.
Serves as liaison and support to community chaplains and spiritual counselors.
Maintains records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
Documents direct services and ongoing communication with community chaplains.
Attends patient care conference and interdisciplinary team/group meetings as a member of the interdisciplinary team/group.
Provides consultation, education, and support to the interdisciplinary team/group on spiritual care.
Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
Provides for funeral or memorial services for patients as requested.
Participates in bi-annual memorial services to meet the needs of personnel, volunteers, and community clergy/spiritual counselors working with the hospice team.
Completes daily logs and time sheet summaries correctly, legibly, thoroughly and on time.
Participates in rotating on-call coverage through the week and on weekends.  

Qualifications
 
Ordination by an organized religious body or ecclesiastical endorsement
One unit of CPE required, 4 units preferred
A Master’s Degree in Divinity, Theology, or an equivalent
Must live within a 30 minute drive to the Abbey Hospice Inpatient Unit in Social Circle
Two years experience in hospital or hospice pastoral/chaplaincy setting preferred
Ability to provide pastoral care and counseling as an interfaith chaplain dedicated to serving the spiritual needs of all those who seek guidance in the hospice setting
Current Georgia Driver License and Insurance
